ClearCare joins CommonWell Health Alliance, shares a vision for improving the aging experience

San Francisco, CA and Boston, MA., – June 22nd, 2016 , ClearCare Online, a software technology platform for the home care industry, announced it has joined CommonWell Health Alliance® as a General Member to help break down barriers to nationwide data exchange, improve the aging experience, and lower the cost of care for our aging population. 

According to Black Book, 94 percent of America’s providers, health care agencies, patients and payers persist as meaningfully unconnected.[i] In an effort to improve the status quo, CommonWell and its members have committed to using their diverse experience across the healthcare continuum to produce and deploy real-world, cross-vendor interoperability services that will contribute to improved health care delivery and outcomes for providers and the people they serve. 

“The home care industry is still largely decoupled from the traditional healthcare ecosystem. And yet, home care has a huge role to play in helping seniors age-in-place and to achieve better outcomes at lower cost and better quality of life. A key to making home care part of the healthcare ecosystem -- part of the continuum of care -- will be interoperability and the seamless flow of data between providers. Because ClearCare works with six of the largest ten home care enterprises and over 2,500 home care agencies nationwide, we believe it’s our responsibility to assist our customers with the connections to the broader healthcare ecosytem.” noted Geoff Nudd, Founder & CEO of ClearCare. 

A year prior, ClearCare, Harvard Medical School, and Right at Home engaged in a 3-year research study to examine and improve aging. Geoff added “Information can flow the other way also -- from the home to the provider.  For instance, the study we’ve pioneered with Harvard Medical School and Right at Home calls for change-of-condition reporting from the home by the caregiver, and escalation back to clinicians as appropriate.  This kind of simple protocol can avert acute, high cost interventions.” 

CommonWell and its members are actively deploying these interoperability services, which are now live at more than 4,700 provider sites nationwide, ranging from large acute hospitals to rural specialty practices. CommonWell services include person enrollment, record location, patient identification and linking, and data query and retrieval across the health care continuum. Unique to CommonWell, its services aim to solve the patient linking and identity problems at scale. Additionally, CommonWell members have access to a working, functioning record locator service, allowing providers to find health records associated with a patient, no matter where the care is delivered. Services are built into the software by CommonWell member companies to help providers maintain their existing workflow.
